Text fields are not included when printing submission

Hello JotForm Person :0)

We use JotForm for our Enrollment at our school.  For some reason, the entire form is not printing out.  We need to have complete copieis in our children's files for the State of NC.  The part that isn't printing is toward the bottom and is the "CONFIRM AND SUBMIT" part of the form that contains important information and agreement that specific forms have been received and read.

"Confirm and Submit" (which has 2 parts that are not printing) is right between "How did you hear about Asheville Montessori School" and "Name of person filling out this form"  Again, there are two sections between those two titles that are not printing.  What would be the reason this particular part doesn't print out for our records?

I went back again and the show header and text box was not check. How do I save it. there were no instructions on the pdf link
