Every time I try to create a new form, the page opens up blank. I'm using a Mac and have tried both Safari and Chrome. The same happens if I try to open an old form to edit it.

I'm sorry for the trouble caused to you.
I did try to create and edit form in Mac and I am unable to replicate the issue you are having. I am not sure what is causing this issue for you but it seems to be specific to your account.
I am reporting it to our backend team. We will get back to you as soon as we have any update from them. For now, I would suggest you to switch to version 3 of form editor and you should be able to edit your forms. Please check the screenshot below on how to switch to old version from your account setting page:

Thank you for reporting this issue.
You can still use the new builder and if you ever come onto a blank page just apply a hard-refresh (cmd shift r) then the form will be loaded without any issues. But if the hard-refresh does not work, please let me know so I can forward it to our developers.

Chiming in to say that I have the same issue with the v4 Form Editor in one of my browsers at the moment: Mozilla Firefox 51.0.1 (32-bit) on Windows 7 OS (64-bit). Once I click on "Edit form", I get redirected to a blank page. Refreshing via the address bar doesn't fix it, but the hard refresh (ctrl+shift+r) did. Guess I'll have to remember that keyboard shortcut.
I just updated to Mozilla Firefox 52.0 (32-bit), and this did not resolve the issue.
Thankfully, using Jotform in Chrome version 56.0.2924.87 has been fine, hence why I hadn't said anything yet. - ET
