When trying to access the form I get an error message saying it is unavialable both with the link and from your page as user.
Please advise what is the problem.

I checked your form and it looks like it was manually set to disabled:
If you would like to enable it again, while editing your form, click on the preferences tab, then set the form to "Enabled".
