The form label and the question are the same. If you have the question set for the field, it should be displaying in the selection. I have cloned your JotForm 50847523988167 to my account and see that the form labels are set correctly. However, they are displaying blank in the email setup wizard.
It looks like there is an issue with displaying the form fields in the email.
Let me forward the issue to our backend team to take a look. Once have any information in this regard, you'll be posted here. We are sorry for the inconvenience this may have caused.
