Email setup wizard: Form fields are displaying blank

    Is the form label different from the question? I don't see any options at all that are called "field label" and I am having the same problem with blank selection options.

    The form label and the question are the same. If you have the question set for the field, it should be displaying in the selection. I have cloned your JotForm 50847523988167 to my account and see that the form labels are set correctly. However, they are displaying blank in the email setup wizard.

    It looks like there is an issue with displaying the form fields in the email.
