Need to have a surcharge for international addresses (outside of US).
Can I use conditional logic so that if country is not equal to United States then add extra cost to shipping amount? Can you show me how to do that? Or is there a better way?

This doesn't help. I don't want to configure a surcharge for every country. I want to add a flat-rate shipping amount for any country that is not the United States.

This is not possible to pass a calculation value to the payment field unless you use a 'user defined amount' payment type.
Alternatively, you might consider adding a separate shipping product which is required and selected by default. Please check the next sample form.
