I embedded the source code for my survey into a Mail Chimp campaign, but it didn't work properly. I lost the radio buttons and text fields.
Most of it is there, but the radio buttons and the text boxes are not showing up. I copied it exactly as it was given to me (source code), but it doesn't seem to be working properly. Is there something I'm not doing right?
I attached a screenshot of what it looks like in Mail Chimp. As you can see, the buttons and text fields are gone.

Please try with the Iframe code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
Before you embed the code, paste it in a text editor like WordPad, you will notice two part of codes, only copy and embed the Iframe part, example:

Thanks for the response, I appreciate your help. Unfortunately this isn't working either. I separated the iframe code as suggested, but when I embed it into a Mail Chimp campaign, literally nothing shows up. The template is completely blank even though I embedded the iframe HTML. Here's a screenshot:

The Iframe code should be read, please contact your MailChimp support, because according to this guide, it is possible to embed HTML code: http://kb.mailchimp.com/campaigns/ways-to-build/paste-in-html-to-create-a-campaign

Yea that's exactly what I did, not sure why it isn't working. I don't get support with Mail Chimp because I don't have a subscription. I'll probably just do an email with link to my survey. Thanks again
