When I check "Keep selected order," the selected order is not being kept. This worked previously, and does not now.

There seems to be issue with the Form Builder for the Visual Multi-Select widget.
I have forwarded a ticket to the next level support.
For now I suggest you switch back first to V3 of the Form Builder and modify/update the widget. Save the changes, and then you can switch back again to V4
I did this test using the Form Builder V3 and it has worked. When I select items from the left, they were added on the right as is without ordering.
Let us know if the temporary workaround switch did not work for you.
