How do I customize the form to add more fields and background?

The purpose of this form is for applicants to apply for funding for their retrospective sport and also for people to apply to participate in youth at risk programs.

Application Form with all applicant details need to include:

Name, Surname, Address, Contact Details, Parent 1, Parent 2 details, Program, Amount being requested, current club details if applicable, Signatures of both the applicant and parents, Disclaimer for authority to publish recipients name and or story on the website. In 150 words or less explain how these funds or program will help you.
