attached is a screen shot of the updated form I have moved the services required up. I have also added a roll over on the additional information none of them are visible on my site. I did replace the code too.

I have checked your website but I couldn't find any form in the link.
Please share the direct link of the webpage which your form was embedded into. After that, we can compare the form (embedded form) in your website and the form (31670276164959) that you provides a screenshot of it.
You also claim that changes on your form is not reflected in the preview. I have tried to reproduce your issue but I couldn't replicate.
