Is there a template with cascading dropdown fields?

    I looked at the widget and it will meet my needs, I wondered if there was already a form template available so that I would not have to start from scratch.  Thanks!

    You may look to our templates page where you could find one that meets your needs and simply make the changes that you need to get applied to it.

    You could then add the Dynamic Drop Downs widget that will help you to have cascading dropdowns in your form.
