Can I use JotForm for a popup window on my website?

Hi.  I was given your name from FMG Suites.  They currently created and managed our website.  We would like a box that pops up that when individuals search and visit our website, www.westsideim.com they are prompted to add in their name and email address.

How long would it take to create this and the cost?
