Can I create an interactive quiz?

I have a PDF Quiz I have created and I would like to make it possible for the end-user to fill out the numbers on quiz then get a total (see attachment).  The answer key is provided to them.   It is intended to be used as an opt-in, so I would locate it on a secret (unsearchable) page on my website (Weebly).  

 

I would like to keep the feel and look of my document.  Can I do this through JotForm?  I already use Jotform for other questionnaires and I' love it.

You can make this happen by using conditions and matrix widget. I created a test form. You can display my test form via this link.I have used "matrix field widget" and s"how/hide field conditions" which are depending on the calculation field.

I have tried to make this work for HOURS and just can’t do it! What I see on my screen does not match what is on the various tutorials, particularly the calculator field. Are you able to see my form? https://www.jotform.com//?formID=70307219992965
It won’t calculate.
What I what to do is get a total of all the numbers added up. You can only select 1 per row, I made all the rows required fields. The order goes: no-0 seldom -1 often -2 frequently. So I just need the sum of all the numbers to appear in the calculator.
Can you help - I urgently need to finish this. Thank you.

You calculation is not working as expected because the conditions you designed have missing fields

Please fix those missing fields with the fields that exists on the form

If you still face any issue, let us know a brief description about what you are trying to achieve with the conditions.

Where do I find the Conditions you noted below? I went into
“edit form”went into the section where the questions are, clicked on the little gear box in the upper right (see screen shot below) and the MATRIX heading comes up, below that are: General / Fields / Options / Advanced and none have the “CONDITIONS” noted below.
I’m so frustrated. Thanks for your help.

Navigate to Settings > Conditions

Hello,
writing back again - still no success and I’m so frustrated!
I literally need a step-by-step instruction. I don’t understand programing, I don’t understand how to operate the form widget calculation (string/number/field).
I have 20 questions (20 rows); I want to a assign a 0,1,2,3 value no=0, seldom=1, often=2, frequently =3; they can only choose 1 option per row; after they complete the 20 rows, I want their to be an automatic sum of all the numbers for each row. That’s it!
I tried cloning one that someone sent earlier, but the cloned version didn’t keep the calculations in place.
I went to the conditions setting but I DON’T SPEAK LOGIC! sorry, this is just alien to me.
There has to be a simple way for a novice to be able to use this. I have other questionnaires that I would love to use that calculation widget with, but unless it’s explained to me in a really simple way, I will remain challenged. Please help.
