How to create a form which can act as similar as e-commerce?

I would like to create a form that can act as similarly as e-commerce. Like if we select item it will appear the price and it will appear the total amounts. Plus, I don't need any payment gateway since I'm going to use an offline transaction credit card authorisation, so there will be a few field for the customer to fill up their credit card details. Please help me.
