    Form is broken. Shows as disabled even though it is not.

    I have checked the status of your form, and I see it got suspended by our automated phishing detector tool. Use of words such as "password" and "credit" is what prompted the phishing detector to flag your form.

    I have just reactivated your account and whitelisted the form, but in order to avoid such possible automatic suspension in the future, I would recommend using alternative phrasing without mentioning passwords.

    For example, you may want to try using a phrase such as "publicly accessible YouTube or Vimeo Channel", and not mention passwords.
