Assistance with migrating or converting PDF to JotForm?

    Hi,
     
    Would you please assist me in migrating or converting the linked PDF to JotForm?  I'm not sure if the URL will still work (the one that I entered when submitting this so I've included the following link to view the form, just in case: https://drive.google.com/open?id=0B8E5NEgSMODLQjhXaXZIcFNLaFU
     
    I already tried importing the form as a PDF via the jotform website; I received an "Opps Import failed" email.
     
    I spent so much time creating the content on my application form...  I can't bear the thought of having to re-create the form from scratch.
     
    The only revisions that I would like to occur (if possible) are:
    -I would like the words "Job Title" at the very top of the form on page 1 to be changed to "Mobile Groomer".
    -change the vertical formatting for the holidays list and days of the week to a more compact layout...   utilize more horizontal space 
     
    Also, where are the forms stored or hosted if I am not embedding on my company website?  Your server?   Cloud? ...Google Drive?
     
    I created the form back when I had a Hireology.com account/membership (last year)...  Just before my membership expired, I downloaded/saved it as a PDF since I was unable to afford renewal of my membership.  When I had an active membership, I was able to customize the form (the attached PDF) via whatever software is used/built into their platform...   it allowed me to share a link to the form on social media (or anywhere I desired) and whenever an applicant would click the link and fill out the form, when I would logged into my Hireology account, the completed form would appear along with the applicants resume and cover letter which were uploaded/attached to the application form before submission. I also received email notifications when a new applicant completed the application form. 
     
    Would you please help me convert my existing form (in the fastest,best way possible) so that it works similarly…?
    Ability to generate a shareable link for posting on social media so multiple applicants can click on the same link to apply for the job… Ideally each applicants responses would be viewable in the form of PDFs, that automaticlly get saved to a specific folder in my Google Drive...
     
     I'm open to other suggestions that don't cost $ and will meet my needs as stated above.

    I posted the above and just wanted add that I saw the following response on a different thread similar to mine...  the info posted there is of no help in my situation...

    The email address used to send the notification may not be monitored, If the PDF form that is being uploaded is complex, it may not be possible to convert to fillable PDF sometimes. If you are trying to convert a PDF form to JotForm, please try using the fillable PDF creator below:

    Hello Lindsay,

    I'm afraid your form can't be converted using our tool into a web form. The PDF form you shared with us contains static text without any fillable fields. Note that the fields should be fillable so it will be converted into a web form. 

    I'm afraid you will have to create the form from scratch in the form builder because our teams are busy working on v4.

    Once you have everything setup, you can integrate your form with Google Drive http://www.jotform.com/help/192-How-to-Integrate-a-Form-with-Google-Drive. This will send a copy of the uploaded files and the PDF submission into your Google Drive folder.
