My form is disabled for no reason

My form has been going in and out of "disabled", even though I'm not setting it to disabled myself.

I checked your account and it was auto-suspended. And this happens by our anti-phishing system that automatically detects and disables the suspicious forms. I reviewed your forms and found that the form is asking for instagram username.

I am re-activating your account but you need to immediately delete/modify the suspicious field or you can delete the whole form. Your account might be suspended again if you don't do this immediately.
