How to use Custom Domain with my forms?

I have set up a custom domain to white label my forms.

Where do I place my custom URL in JotForms, so that my forms go out with my URL?

My Custom Domain: forms.thehealthycarguy.com

I can click on this custom URL it goes to my Jot form site, how do I get my forms to use this URL.

You have to add a DNS record that will point to your custom domain.
