How to create paying form with logo of my company?

I created a page with help from a friend when I had my own website over a year ago.  Now I need technical support to add an information form with clients contact information implemented on their websites.  It doesn't seem you have a phone number to actually talk with someone.

Can you have someone call me? I do much better with verbal communication on technical issues.  In the mean time I'll Youtube

I reviewed your form and that is a completely fine contact form. You can embed your form in the website in several ways. You can refer to this guide https://www.jotform.com/help/67-Which-Form-Embed-Code-Should-I-Use on how to do that. But we always advise iFrame method as the best one: https://www.jotform.com/help/148-Getting-the-form-iFrame-code

You can create a paying form by using one of our Payment integrations: https://www.jotform.com/help/323-Mastering-Payment-Form-Integrations-with-JotForm

Regarding the logo, you can use Header field:

And add the logo image.

Or you can use the Image field for the same purpose:

I have a client that wants me to recreate a form similar to this fashion
and have the embedded code placed onto his website. Can this process be
done on the Jotform templates?
If so can you send me a tutorial on how that works. I'm due to talk with
