How can I create a monthly subscription form?

$50 per month for a specialized website.

It is  a subscription form we willbe putting on a web page. The page has not been designed yet.

For subscription forms, you will need to make use of a payment field and use the subscription method of payment. Also, you will need to have an account in one of the payment gateway services available that we use for integration such as Paypal, Authorize.net, Stripe, etc. 

Once you have your account setup with the form, all subscription payments you receive via your form will be directed to the payment gateway and your account under the payment gateway service will start charging your subscribers on a recurring basis.
