I have checked the status of your account and found out it was automatically suspended due to suspected phishing activity in your form. Our anti-phishing tool that scans each form created in Jotform has detected credit card fields in your form and flagged it. Once a form is flagged for suspected of any phishing activity, the account that owns the form will be automatically suspended because it is against our terms.
I have removed the fields from your account and reactivated your account.
As we do not encourage collecting of credit cards within form fields, how would you like to consider trying any of our Payment Integration instead? You can also allow or enable payment authorization in most of our payment integration. This option allows you to collect payments at a later time.
