My form on edit mode is okay, however when I view my form and/or click the link, the margins do not synchronize

The format on edit is okay, the margins are aligned. However when viewing the form, the the date box is misaligned and is not on the same row level as the rest.

What's happening is that your date field is trying to merge with the above line.

Please edit your date field and click the down arrow, just beside the properties icon, and select Move to a new line

Once done, save your form and try again to preview the form.
