Would you mind sending us your external IP and DNS details? This would help our developers during their investigate.

The issue seems to still persist due to an e-mail provider issue but you can try to visit your form using following link as a temporary solution.
Once the issue is fixed permanently, you will be notified again.

We have an update regarding this issue.
form.jotformeu.com was blocked in Turkey but the issue is fixed now. If your internet service provider is TTnet, you can view your form now but you may need to wait at most 24 hours for other internet service provider.
Please let us know if the issue still persists.
