I have check your form and I could not replicate the same issue. Your form is working properly on my side. As you can see:
Also, I have checked your account for seeing your Usage. Everything seems to be good, only total form limit is full. However, it does not block your current forms. Only thing, you can not create a new form. You must delete your current form for to create a new form.
I have cleared your form caches. Please clear your Browser caches and try to view your form again.

Whatever you did, it works fine for me now.
Hopefully non of the recipients will have any problems also.
