The number of forms limit- is that per month? per year?

I send out a monthly form, but was recently prevented from creating one.  Am I expected to pay $99/month for 1 form?

The forms limit does not reset, if you would like to create a new form then you would need to disable one of those forms you no longer use in order to get this free space and create  a new one. 

Your account is currently on the bronze plan so you're able to have active up to 25 forms now, if you purchase the gold plan then you will be able to have unlimited active forms.
