My Forms Keep getting disabled!!!!

    I am designing a form for my company to get information from vendors. When ever I design a form I am not able to preview it because the form is disabled. However, whenever I look into the form details it is in fact enabled. I need someones help on why this keeps happening?

    Our phishing detector disabled your account, however, I checked your forms and they look to be within our terms of use.  I am not sure why our system was interpreting them as phishing forms.  I whitelisted your forms and activated your account, your forms should be usable once again.

    My forms seem to be disabled again and I cannot seem to find the problem.
