Nowadays, we have some issues with Turk Telekom. Because of this reason, you encounter this problem. Our developers have been already working on it.
Could you please try to view your form without "eu"?

We have an update regarding this issue.
form.jotformeu.com was blocked in Turkey but the issue has been just fixed. If your internet service provider is Turk Telekom, you can view your form now but you may need to wait at most 24 hours for other internet service provider (such as Super Online etc.).
Please let us know if the issue still persists.
