I am unable to log in to my account on other devices:
It was suspected to be used for phishing. If you think this is a mistake please contact us to resolve this issue.Is this why I can't see any of my forms?I just get an error message:"Form not found. This form is disabled."Thanks, Belinda

Your account has been auto-suspended by our Anti-Phishing Tool. That's the reason why your form has been disabled. Please note that asking for credit card details, passwords, logins, usernames, etc from the users of the form is against our Term of Use.
I went over your form and I noticed that there were fields that have a "credit" and "card" words on it. If you want to ask payments from the users of your form, you can use the available payment processing gateways supported by JotForm. Payment Processing Apps.
Please inform us if you have already removed the guilty fields from your form and we will activate your forms right away. If you cannot remove it by yourself, we can manually remove the fields for you.

I cannot make any changes to my existing forms because my account has been auto suspended and I can no longer log in.
I am not taking any payments on my forms and do not want to receive any credit card information at all.
This must have something to do with templates from your website that I was playing around with, purely from a design point of view. You are welcome to delete any fields or forms that are currently on my account and I will start again.
Please unlock my account or let me know what I can do to begin working again.

I have reactivated your account after reviewing all of the forms in your account including the once in the Trash Folder.
Like what my colleague have mentioned above, your forms were marked due to the "credit" and "card" text(s) in you form(s).
Since I see that you have only used those words to gather a tip amount from your respondents, I have reactivated your account. However, there is no guarantee that this would not happen again. And I would suggest that if you could use any substitute/other word(s) for "credit card", then please do so.

Hello, I have been locked out of my account and I now realize why. It's because I was trying to create a Credit Card authorization form. I have now found another solution where I don't need to create that form and just want to delete it and be able to get back into the account.
Can you please fix it and reactivate my account?
