Forms are not working any longer

    I don't understand why this form is not working:

    Can you please fix this. It looks like all my forms at not working now.

    It says: 

    Form not found
    This form is disabled.

    Upon checking that form and your account, you have been autosuspended. 

    You are using plain text boxes on collecting sensitive information like credit card details:


    Please note that we do not allow users to create forms that manually collect this kind of information. This is against our Terms of Use. I have quoted it below for your reference:

    "Scams. You agree that you will not use JotForm or any form(s) you create using JotForm for scamming purposes to attract, lure, or illegally obtain payment of any sort from others by unjustifiable means such as posting a jotform as a money-making program on a classified ads site as an example of that. Any account reported or found doing so will be immediately suspended.   

    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information."


    Violation of our terms will most likely lead to account suspension. If you wish to collect payments from your users, it is a must to use our payment integrations. To learn more about it, you can check the following links:

    I am also not user if you are directly related to Intuit QuickBooks or you are a reseller. I see that you are using its logo and name on your form:


    I suggest you add more information on your form like if you are a reseller or a reseller badge if there are any. Our dedicated review team might think that your form is mimicking Intuit Quickbooks official website page and might be used for Phishing. 

    If you agree with our terms, I will need to delete the credit card fields that are guilty and we will re-activate your account by then. But you will update all forms that you see have violations and avoid creating similar ones again.
