I have checked your account and I have seen that your account has been auto-suspended due to phishing as you have requested credit card information from users without using any payment gateways as the following screenshot of your form named "Payment Information [Navigos]":
This situation violated our Term Of Use and your account has been suspended. This is the reason you can not reach any of your forms and they seem to be not working.
I had to delete those forbidden fields from your form and re-activated your account again.
