I'm having problems with the PDF customization

    I have over a dozen forms - all that begin with the word 'Spring'.  In each form, I needed to customize the .pdf as this functions as a printed registration form that our users will sign after they get it in the auto email.

    I have no problem adding a text title to the page, but every time I attempt to add an image from a URL, the image ends up either replacing the title, multiplying itself or disappearing completely.  I can either add an image OR a title, but not both. 

    The logo URL is:

    Can you help?

    I cloned your form and I was able to add an image or a text in the PDF Designer. I was able to resize and move the fields as well. Here's a screen capture:
