I have a user who can't submit our recommendation form through the website. Here is our url. The applications are going through just fine, and I can see other have submitted the recommendation form without a problem. This is the third person in the last two weeks though, so something is wrong.Page URL:

I could not reproduce the problem. It worked for me when I upload files into your form.
You can ask the users to try it again or reload the browser when the upload fails. Sometimes this could also be a browser or network problem. If the user's network isn't reaching JotForm servers, it might result in upload failure. You can suggest using different browsers too.
