I did check your account and did not find any any issue with that. Your account is already active and it should work fine.
I would suggest you to please test your form again and get back to us if you have any problem. The only issue I found in your form " CCBA Membership" is that there is no submit button. I would suggest you to please add submit button in form. Please check the screenshot below:
