You can attach a file in the Autoresponder email. The Autoresponder email is the confirmation email that will be sent to the user who completed the form. Please refer to the guides below:

    Setting-Up-an-Autoresponder-Email
    How-to-Attach-a-File-to-Autoresponder-Emails

You just need to create a condition if the Autoresponder email will be triggered or not. You can use the Change Email Recipient condition for that. Here's a guide: How-to-Send-Email-Based-on-User-s-Answer.

You may also want to check our Quiz App. Related guide: Turn-your-form-into-a-Quiz-or-Exam.

    Hope that helps. Thank you.

    Hello
This would require an email and some of the employees don't have that. I'm looking for it to auto populate a certificate with their name etc so they can print right then and there. If they are not healthy enough to pass and answer the questions that pertain to this, I want to populate a denial based on answers.
    Thank You

You may also consider customizing the Thank you message so that it would be displayed on the screen after submitting the form.

You may display different Thank you messages, based on the result by setting up conditions on the form.

Hope this information helps! If you have any other questions, please let us know. We will be happy to help.
