I'm going to create a medical questionnaire and wanted to know if I could auto populate a certificate of completetion if they pass and how would the.

You can attach a file in the Autoresponder email. The Autoresponder email is the confirmation email that will be sent to the user who completed the form. Please refer to the guides below:

How-to-Attach-a-File-to-Autoresponder-EmailsYou just need to create a condition if the Autoresponder email will be triggered or not. You can use the Change Email Recipient condition for that. Here's a guide: How-to-Send-Email-Based-on-User-s-Answer.

This would require an email and some of the employees don't have that. I'm looking for it to auto populate a certificate with their name etc so they can print right then and there. If they are not healthy enough to pass and answer the questions that pertain to this, I want to populate a denial based on answers.

You may also consider customizing the Thank you message so that it would be displayed on the screen after submitting the form.
