Is there a 3 Column Dropdown User Contributable Widgit?

Contact, Address, Telephone

 

Looking for a Drop Down Widgit that allows 3 columns (Contact, Address, Telephone)

Where the user can either select one of the pre-defined drop-down options OR to have the ability to enter in another (Contact, Address, Telephone) as a new option?

I see that you're looking for some sort of custom 3 column user-contributed dropdown usage.

 

The closest I'm aware of as of now to something like that would be these two widgets:
