I am following the described process to add a form to my facebook page.
unfortunately there is a step where i have to chose the page i want to add the form in a scroll down menu, i see 2 of my pages but not the third one (vision canada immigration), the exact one i need to insert a form right now

Is it possible that the account you are logged into does not have admin access to the facebook page you wish to add?

Thanks for your reply. But I don't believe it is an admin rights to access my pages.
I only have one Facebook personal account
I created from that account 3 pages (only 2 appear when in jotform)
I manage those pages from inside my only personal account
if it wouldn't work for any of them, I could make sense of it. Cuz all pages have been created the same way and accessed the same way for updates
I checked our integration with Facebook and it is working properly. I can see all my forms from the left panel on Facebook.
Please hard refresh your browser and try again. Use cmd+shift+R for mac and ctrl+F5 for windows.
You can also insert your form from JotForm builder. The step you refer where you choose from your forms is not present on this option. Here is a guide on how to do this:
