    Hello juan7707,

It seems you must have edited your email alert and then made the changes in your form. Please note that if you edit your email alert, any changes you make in your form will not reflect in email alert.

The best way to solve this problem is to delete the existing email alert form your form and add a new one. A new email alert will always include all your form fields. You may like to take a look at the following screenshot on how to delete email alert and add a new email alert:

    Hope this helps.

    Thank you!

I found the missing field was fixed with redoing the email, but the problem with the widget still persists, after inspecting further the widget seems like it is happening with this is an isolated issue with this specific widget.

I tried added a new one with the exact configuration and it worked fine:

May be possible for you try replace the widget with a new one?

If so, please try and let us know if the problem persists.
