    I was unable to find the files on our servers, the only one that seems to be properly uploaded is this one: Hispeed Drug Free Workplace.pdf 

    I have also tested your form and all the files were submitted correctly, they are available in the submissions page. 

    I would suggest you to ask your submitter to send the files again and ask to remove  spaces from the files names, you could provide the edit link for that submission so he / she does not have to enter all the info again, to do so simply replace the highlighted part from the link below:
