Hi, I went to login to our Jotform account and received a popup stating the account has been suspended for violation of the Terms of Use.

Do you know what this could be? We have used Jotform for quite some time now and have never misused it in any way.

I need to get into our account.  Please help.

    Thank you.

    Hi Slade,

your account was automatically suspended due a "possible" pishing form.

In fact your form 21226013160940 contains only one password field (ftp password).

However, I've checked all your forms, and I can see that you never scamed but you're serious and professional.

I've whitelisted all your forms and activated your account again.

Please note that according our t.o.s. it's prohibited to use a "password fields" in order to avoid pishing forms.

BTW your account are back and you can work again with JotForm ! :)

    Hope this help you.
