Form Field not working in Firefox or Chrome but works in IE

    I asked this before I was told to use iFrame but I don't understand why this doesn't work with just the HTML? All I want is a single page on my site with the conditional filed working. They work in IE but nothing else and I don't understand how to correct it.

    I tested the form https://www.jotform.com/70365135958160 in Chrome and it worked fine.

    While trying to test the form at https://myvoiproute.info  I could replicate the issue.

    This seems to be issue from the clodflare services

    A Parser-blocking, cross-origin script, https://ajax.cloudflare.com/cdn-cgi/nexp/dok3v=f2befc48d1/cloudflare.min.js, is invoked via document.write.     Please try using the iframe code and let us know if that fixes the issue

    Yes that's true it works using https://www.jotform.com/70365135958160

    and now with CloudFlare disabled it does work at https://myvoiproute.info using all other browser s and I have tried 5 other browsers. I will try and figure this out with CloudFlare and regarding iframe I will test that on another domain I have to see how that works

    I tested your link, and yes, it does work. 

    Please check with CloudFlare if they do some kind of blocking that could cause this problem.
