Hello ESprotte59,
You have created two forms under a guest account. This is a reason why you are not seeing the forms in your existing personal account. As I see that the guest account and your current account have used the same IP address, I gave the benefit of the doubt and transferred the forms.
Please check again your My Forms page and let us know if the problem persists.
