Form is disabled

    I just signed up for an account, made my first form and I can't preview it. I made a separate form using a template and didn't change anything and that one also says form is disabled. I did look in to that and it seems like that means you've gone against the terms of use and I don't see how.

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

    The form http://www.jotform.us/form/70375316502147 is collecting Credit card information using plain text field which is not allowed with JotForm. If you need to receive payments, it is required to use one of the payment methods available in the form builder. These payment methods comply with the PCI/DSS security standards to process the payments securely.  

    I have deleted the guilty fields from this form and activated your account. Please refrain from adding such fields to your forms to avoid any further suspensions on your account.
