My contact form worked during a test run before creating the unique url. However, now whenever you try to press "Submit", it gets stuck on a "Please Wait" notification. The link was copied and pasted onto a .pdf flyer (attached) and shared as an email attachment.
Any suggestions on why the form is no longer working? Please help!

I was able to replicate the issue using your form's direct link. I was stuck at "Please wait" button after hitting the Submit button. However, it is working properly on a cloned form.
Please try disabling the Send Post Data in the Form Settings:
