Why is my form disabled?

    I just made a form and tried to view it but it says its disabled? why? and how do I fix this?

    That is because your form is asking for credit card details using our plain text fields. We do not allow such forms. It's against our Terms of Use for Phishing.

    I have reactivated your account, but I have deleted the CC fields.
