Can I make the recipient complete a portion of the form and return it to me?

We want to have a supervisor complete the main body of the form, then submit. Upon submission, we want a copy to go to the person supervised, have them complete the bottom portion and upon submission of their part, have the two parts return to the office file.

Is it possible to make a two phase form?
