I am unable to add my form to Tumblr

i am unable to add my form also. it shows up in the preview on tumblr once i add it and looks fine but after its saved and i go to the page and click the link i get an error message

Can you please provide the URL of your page where the form returns an error when you click its link? It'd be great if you could share the screenshot of the error you're getting. We can then further investigate what's wrong with it.

hey sorry it didn't attach. i can attach a file and give you the website URL. the URL I am having issues with should be the order link on my page jackiebristow.ca

so please go to my website jackiebristow.ca

then click order at the top

i didn't post the URL to the order page because for some reason it is not working right. The URL for the order page that is not working is www.jackiebristow.tumblr.com/order 

but when i click order on my website jackiebristow.ca the uRL that shows up is www.tumblr.com/order ??

so i don't know whats going on i have attached a photo so you can see that along with the error message. 

I was able to see the same error when I connected your order page. However, I tried to embed one of my form to my tumblr page with Iframe method and link method, it worked and I was able to see my form on my site.

Could you please confirm which method you used when you embed your form to your tumblr site? If you use link method, please be sure that your url is correct. 

If you embed your form via  text method, please try to re-embed your form with Iframe method.

ok i followed it but I'm still getting the same error :(

ok os i finally got it to work but its cutting off the top part of the form as you can see here

i also can't put anything else on the page which is what i wanted to do. I wanted to have my order information above the form but it won't show the form and i get the same error when i try to do that

I have checked your web page and see that the width of the wrapper that is embedded with JotForm is lesser than the width of the form.  Could you try decreasing the form width to 600px to see if that works?

Also, you may need to increase the height of the iframe in the code in order to display the complete form on the web page.
