Why do my forms keeps getting disabled?

Everytime a make a form and add it to my website it gets disabled.

We're sorry for the inconvenience you have.

I have checked your account and I have seen that your account has been auto-suspended by our automatic anti-phishing detection system as you have requested username from users as the following screenshot:

You are not allowed to collect any kind of credentials in your forms. So that, this situation violated our Term Of Use. 

So, I had to delete this forbidden field from your form and re-activated your account again.
