Show headers and text in PDF submissions.

I have created an application on JotForm. A part of the application is a consent and release that I have typed in a text box with digital signature underneath. I need the submission to include the text box text as well as the digital signature. 

The current submissions are being reformatted to where I am only seeing the signature and date but not the words. Ideally I would just like a PDF of the form exactly as it is on the website without any reformatting.
