I'm trying to log into my account and it states that it's autosuspended. What does this mean and how did this happen? I've never experienced this before. In addition, my forms are not loading on our website. Please assist with this issue ASAP. Thanks.

I've taken a look at this report about your account being suspended but, I looked at your embedded form on the page you provided I found I could see it working normal.
Perhaps this was a mistake and fixed by one of our team members or was previously sorted out.
If it's still a problem on your end though then please do let us know.

My account has been autosuspended again and I really need to work on these forms for my client and deliver to our developer. As stated before, We're not going to use the form as I've designed it. I've designed it as an example for our Developer to use the embedded code and input our Authorize.net to function as the credit card merchant.
Your quick response to this would be greatly appreciated. I just realized that your team comments to the post itself and not via email. Cheers!

Hello Larry,

Your account was auto-suspended because you were asking for credit card information which is forbidden by our Terms of Use.
I removed all those fields, please use one of our Payment integrations if you need to collect payments. And I did that for both forms:
Please avoid putting those fields back or your account will be disabled permanently.

Thank you. Will the way I have the form work now without suspending my account? As stated before, our Developer is only using this as an example to build internally. He will take the code and properly set-up our authorize.net for fulfillment. Thanks and please advise. Cheers!

If you don't put those fields back it will work and it won't be auto-suspended. And if you use payment integration fields you're safe.
Something more, please remove the password field as well since it is not allowed to collect that type of data:
