I have set up my form on my website. If using a computer, the form comes up perfectly. When trying to access it from a mobile device, the screen comes up blank. I have set the form to "make the form responsive". What am I doing wrong?

I checked both the the form in your account on Android mobile. I did not face any issue. Bot the forms loaded fine on mobile

I am trying to access it from my Samsung Galaxy 7. I use Chrome for a browser. I have went in and cleared the browser cache. The attached photos show that it still is not showing. Any other suggestions?

Hello Renee,
I checked your URL and I was able to replicate this issue. The form is not showing form either.
This could be due to how the form is embedded in your website.
Could you please try to use this code instead:
